The NMC LOTAG28 lockout tag is made of 15 mil vinyl for water and chemical resistance, and has a red, white, and black "Danger" logo and diagonal red and white stripes on both sides for high visibility. Text on the front of the tag reads, "Do Not Operate - Equipment Lock-Out -Tag & lock to be removed only by:" followed by two blank lined areas labeled "Name" and "Date." Text on the back of the tag reads, "Lock-Out Procedures" followed by a list of 10 common lockout procedure steps. This tag has a tensile strength of 6,300 pounds per square inch to help resist unauthorized removal. It includes a 3/8" diameter hole for fastening and a reinforced brass grommet for longer wear. This tag is part of a lockout/tagout (LOTO) safety procedure required at facilities where a device or energy source needs to be isolated during maintenance and repair. The tag measures 6 x 3 inches (H x W). (H is height, the vertical distance from lowest to highest point; W is width, the horizontal distance from left to right.)
