The NMC LOTAG18 lockout tag is made of 15 mil vinyl for water and chemical resistance, and has a red, white, and black "Danger" logo, "Equipment Lock-Out," and diagonal red and white stripes on both sides for high visibility. Text on the front of the tag reads, "Equipment Lock-Out - A Life is On The Line!" followed in smaller print by "This tag & lock to be removed only by the person shown on back." Text on the back of the tag reads, " Equipment locked out by" followed by four blank lined areas labeled "Name," "Dept.," "Date," and "Time." This tag has a tensile strength of 6,300 pounds per square inch to help resist unauthorized removal. It includes a 3/8" diameter hole for fastening and a reinforced brass grommet for longer wear. This tag is part of a lockout/tagout (LOTO) safety procedure required at facilities where a device or energy source needs to be isolated during maintenance and repair. The tag measures 6 x 3 inches (H x W). (H is height, the vertical distance from lowest to highest point; W is width, the horizontal distance from left to right.) Lockout/tagout (LOTO) safety procedures require that tagout devices meet Occupational Safety and Health Administration (OSHA) 1910.147(c)(5) standards for quality assurance. Lockout tags (also called safety tags) use color and warning text to help prevent machine operation or power activation during maintenance and repair.
